| 401k Account Information (Optional) |
A 401k plan is available (but not required) for every W-2 employee of Murphy & Associates. There are four enrollment periods: January 1st, April 1st, July 1st and October 1st. The plan is administered through EPIC Advisors offering a core menu of 20 different mutual funds. There is also a self-directed account option through Charles Schwab which is not limited to specific funds. Employees can purchase any stocks, bonds or mutual funds that Charles Schwab offers. This document contains more information about how the 401k plan operates. Upon achieving eligibility you will receive an email with an invitation to participate. |

| Participant Distribution Election (Optional) |
In the event you need to withdraw money from your 401K, close your account or roll it over into another account, please fill out the Participant Distribution Election form and send the original to us. |

Complete one of these forms to indicate your desire to either participate in or waive the Health Reimbursement Arrangement plan offered through Array Health Solutions. One form is required – either the Participation form or the Waiver form. |
